Niche Planner Pages created

I'm looking for someone to create printable planner pages for a specific niche for 2016. I want to brand the pages and also add in different motivation for some pages. For now I need in 8.5x11:

*two page monthly calendar page for the 12 months in 2016, with major holidays added
*two page weekly calendar page that they can add the date to. This will have space to write out things to do each day of the week
*general to do list
*shopping list
*address pages
*medical history
*medications - mine and family members
*emergency contacts

I will be looking to offer up different additional printable packs as we create them, plus add a planner for a different niche!
